Thank you for volunteering for an upcoming event. To finish your volunteer signup, please complete the following three steps.

Step 1

Waiver and Authorization form for ALL Volunteers

Everyone volunteering must complete this volunteer waiver of liability, and authorization to conduct a background check.

Step 2

Submit Your $26 Payment for your Background Check

Pay $26 here for us to conduct your background check. If you have a national background check completed by a partner church/ministry within two years of the event you are volunteering for, you may submit a copy of that to maryjo.walters@smcfmc.org. You must still complete Step 1.

Step 3

Complete the Background Check Form

Submit your personal information to “Protect My Ministry” to so that a required background check may be conducted.
